RESIDENT CARE COORDINATOR

Key Responsibilities

  1. Build strong, trusting relationships with residents by meeting with them regularly, engaging in meaningful conversation, and learning their personal history, preferences, and routines.

  2. Use these connections to observe and identify any changes in health, mood, or behavior, and document findings.

  3. In line with these observations, identify ways to encourage resident engagement in social activities.

  4. Write and update individualized care plans that honor the resident’s dignity, preferences, and family input, while ensuring alignment with Title 22, CCLD regulations, and organizational policies.

  5. Take full ownership of care plan execution – personally ensuring all steps are completed as intended. Work hands-on with Direct Care Staff to implement plans, provide guidance, and follow through until resolution.

  6. Reach out to other departments within the organization or outside providers and vendors to support the welfare and social needs of the residents.

  7. Maintain clear and consistent communication with residents’ families, physicians, and care teams.

  8. Lead, mentor, guide, and retrain Direct Care Staff as needed to ensure the consistent delivery of high-quality, person-centered care. Facilitate group discussions or provide one-on-one mentoring and hands-on coaching, model best practices, and follow up to confirm skills are applied in daily routines.

  9. Maintain accurate and compliant documentation in Electronic Health Records (EHR) or e-MAR systems.


Other Responsibilities

  1. Respond to medical emergencies by observing, assessing, and determining the necessary actions in situations requiring urgent decision-making and time-sensitive response.

  2. Lead the assisted living admission process, including initial inquiries, tours, application completion, medical history collection, assessments, and agreement finalization.

  3. Execute the operational plan for handling environmental emergencies. Depending on policies, this may involve coordinating and facilitating the movement of residents, ensuring effective communication, and managing logistics during the event.

  4. Engage in all community activities, including community outreach efforts to build relationships and represent the community positively.

 

 Requirements, Qualifications & Skills

  • Flexible and dependable, able to work varied shifts including weekends and responding to emergencies.

  • Prior Direct Care experience in assisted living, memory care, or similar senior living environments.

  • Specialized dementia care experience with proven resident engagement skills.

  • Software proficiency: Electronic Health Records (EHR), e-MAR, Microsoft Office Suite (Word, Excel, Outlook), and related programs.

  • Leadership and oversight experience, including mentoring and retraining staff.

  • Strong knowledge of Title 22 and CCLD requirements.

  • Exceptional communication, organization, and problem-solving abilities.

  • CNA, LVN, or similar healthcare certification preferred.
